Development of the Transition IEP is a critical link in the planning for the future of students with disabilities as they grow into adults. The following resources, in conjunction with the discussions on Transition Services in IDEA 2004 should give you a good starting point in your transition planning.

 

Resources

The Able Trust provides grant funds to Florida not-for-profit agencies and citizens as part of its mission to provide Floridians with disabilities fair employment opportunities. The Trust is committed to identifying, initiating, and funding new and creative programs to carry out its mission, utilizing existing organizations, associations and agencies wherever possible. Able Trust will only consider funding projects that cannot otherwise be provided for through a state agency.

The Big Bend Transition Spectrum of Services and Guide for Students with Disabilities is a product of the Big Bend Transition Council and is updated each year.  It provides specific information on services and programs for all students with disabilities from age 11 and beyond for Leon, Gadsden and Wakulla counties.  The title above is a link to the document.  It includes many website links.

FYI Transition This web site portal is designed to provide student with disabilities, their families, teachers and other professionals information to guide decision making about a student’s transition from a Florida school program to adult life.

Project 10:Transition Education Network at the University of South Florida, St. Petersburg provides information on resources, publications and conferences for students, parents, teachers and other professional staff.
